Morning Staff Substitution Form

You must still have the signatures of the person agreeing to sub as well as a signature from management.  You must have them sign the confirmation report that comes up after you submit this form.  If you have any questions please call the supervisor of your shift.  Make sure the person you have subbing is aware you have put them on this form.

Evening Staff Substitution Form
